A simple survey app! As of time of writing it only lets you create surveys with free text questions. Other survey creation tools can do a lot more than that. But simple-survey has the advantage of not spamming you with ads, not showing cookie/GDPR popups (while still respecting user privacy), or ask you to sign up. Initially it was created so that we could gather some attendee info before the firstelm-camp. Since then it's been used for the 2024 Elm camp and nothing else that I'm aware of.
